Partial breast cancer op warning | Cancer Kick

Women undergoing treatment for breast cancer should be warned that 20% of patients who choose to only have part of the breast removed are likely to need a second operation, scientists have said.

Researchers made the suggestion after they found that one in five English women who underwent breast conserving surgery needed further treatment.

There are 45,000 women diagnosed with breast cancer each year in England. Of these, 58% chose to have part of the breast removed ? breast conserving surgery ? rather than a mastectomy which sees the whole breast removed.

When combined with radiotherapy, breast conserving surgery produces similar survival rates to those achieved with mastectomy alone. But because some tumours are difficult to detect, breast conserving surgery may result in their inadequate removal and lead to another operation.

The study, published in the British Medical Journal, examined the re-operation rates of 55,297 women who had primary breast conserving surgery in 156 NHS trusts in England between April 2005 and March 2008.

Of these women, 11,032 needed a second operation within three months. Among women who had breast conserving surgery as a re-operation, one in seven needed further surgery, researchers found. More than 80% of the the 55,000 women were suffering from isolated invasive cancer, 12% had isolated carcinoma in-situ ? or pre-cancerous disease ? and 6% had both invasive and in-situ disease.

Re-operation was more likely among women with pre-cancerous disease compared to those with isolated invasive disease. Around 40% of women who had a re-operation underwent a mastectomy. Re-operation rates also varied between NHS trusts ? in some trusts re-operation rates were below 10% while in others they were above 30% ? but the authors stress that further research is required to understand its cause.

Dr Emma Pennery, clinical director at Breast Cancer Care, said: ?Surgery is the first treatment for most people with breast cancer and some people will be offered the choice between a mastectomy and breast conserving surgery. It?s really important that women are aware of all the potential benefits and drawbacks when they make this decision so their choice is informed.

Macmillan Cancer Support head of evidence Dr Siobhan McClelland said: ?Nobody wants to go through the emotional strain of unnecessary surgery or mastectomies. This is why breast conserving surgery has become more popular.

?But it is essential that oncologists ensure that breast cancer patients are made aware of all of their treatment options and their possible results before any decision is made. However, this should not undermine what the surgeons are doing ? it also shows that 80% of breast conserving surgery is successful.?

(AP) ? The Institute of Medicine recommended Friday that soldiers returning from Iraq and Afghanistan undergo annual screening for post-traumatic stress disorder and that federal agencies conduct more research to determine how well the various treatments for PTSD are working.

Of the 2.6 million service members deployed to Iraq and Afghanistan, it's estimated that 13 percent to 20 percent have symptoms of PTSD.

Federal agencies have increasingly dedicated more resources to screen and treat soldiers, but considerable gaps remain, according to the Institute of Medicine, an independent group of experts that advises the federal government on medical issues. Its recommendations often make their way into laws drafted by Congress and policies implemented by federal agencies.

Barely more than half of those diagnosed with PTSD actually get treatment, often because many soldiers worry it could jeopardize their careers. Also, when soldiers do get care, they're not tracked to determine which treatments are successful in the long-term.

The Defense Department provides medical care to active members of the military and the Department of Veterans Affairs cares for those who no longer serve. Sandro Galea, the chairman of the Institute of Medicine panel, said both departments offer many programs for PTSD.

"But treatment isn't reaching everyone who needs it, and the departments aren't tracking which treatments are being used or evaluating how well they work in the long term," said Galea, a professor and chair of the epidemiology department at Columbia University. "In addition, DOD has no information on the effectiveness of its programs to prevent PTSD."

The report concludes only the first phase of the Institute of Medicine study. The panel, which advises the government, is hoping to release a second report in 2014 that will provide more specifics about the number of service members and vets who have PTSD and the outcomes and costs of their treatments.

Nick Colgin, who served for 15 months in Afghanistan as a combat medic until April 2008, suffered a traumatic brain injury after a rocket-propelled grenade slammed into his Humvee. While he showed symptoms of PTSD upon his return to civilian life, he waited to get treatment until just last month.

"I just didn't want anyone to know I had that issue. I didn't want to know myself," said Colgin, 27, now living in New York City and working with the advocacy group Iraq and Afghanistan Veterans of America.

Jason Hansman, the group's membership director, said the stigma of seeking treatment is even more prevalent among those on active-duty.

"A lot of people believe their career will end if they seek treatment," Hansman said.

In a recent speech to mental health provides, Defense Secretary Leon Panetta said commanding officers must make it understood that seeking help for the stresses of war should be seen as a sign of strength rather than as a sign of weakness.

PTSD is triggered by a specific traumatic event, such as being in combat or witnessing death. The symptoms of the illness include a numbing of emotions, difficulty concentrating and exaggerated startled responses to events.

The institute recommended therapies supported by robust evidence, such as working with patients to change their thinking and emotional responses to stress. But the committee's analysis of other innovative treatments, including yoga, acupuncture and animal-assisted therapy, is hampered by a lack of evidence on their effectiveness.

Galea said that the two departments have been under quite a bit of pressure to get programs up and running to deal with the growing number of vets with PTSD.

"There hasn't been as much premium placed on tracking than there has been on implementing treatment," Galea said.

The panel praised the two departments for issuing joint guidelines for managing PTSD, but it's unknown whether their providers adhere to the guidelines. The panel said that primary care doctors within the VA screen Iraq and Afghanistan veterans annually for symptoms of PTSD, and it recommends that the Defense Department do the same.

The panel said it is hopeful that the departments will make more use of therapy through videoconferences that will allow patients in remote locations to get care.

The panel also called for more research to shed light on the brain's defense mechanisms for stress, identify factors that can influence the timing and severity of symptoms and identify signs that could help lead to earlier diagnosis and more precise drug treatments.

The report said that the VA treated more than 438,000 veterans for PTSD in 2010, showing evidence of the widespread scope of the problem. Similar numbers are not available for the Defense Department.

Cynthia O. Smith, a DOD spokeswoman, said the department has already taken steps to address some of the issues raised in the report.

"The department recognizes the need for continued improvements," Smith said.

The VA said it would review the report and noted that it recently announced it was adding 1,600 clinicians and 300 support workers to its mental health staff.

"We have already made strong progress, but we need to do more," the department said in a written response to the report.

(AP) ? A huge solar flare erupted from the sun Thursday, streaming radiation toward Earth.

The solar storm is expected to arrive early Saturday, but forecasters at the government's Space Weather Prediction Center say they expected minimal impact to communication systems and power grids.

It may spark colorful auroras in southern Canada and northern United States.

The storm is part of the sun's normal 11-year cycle, which is supposed to reach a peak next year.

NASA's Solar Dynamics Observatory captured Thursday's flare, wowing scientists with images of the massive outburst.

'Fifty Shades' author delights fans at Comic-Con

SAN DIEGO (AP) ? Lola De La Cruz cried when she met author E L James. The 29-year-old fan had waited since 4 a.m. to accomplish the No. 1 thing on her Comic-Con to-do list ? meet the writer of "Fifty Shades of Grey" ? and 12 hours later, she got her chance.

"The relationship that's in the book is something that I've always wanted and haven't gotten in previous relationships, so it kind of gave me that hope that there's something still out there," she said as she wiped away tears after meeting James Thursday at the pop-culture convention. "It brought me back to life, so to speak."

James can relate. The 49-year-old mother of two was a fan herself, moved to start writing after reading Stephenie Meyer's "Twilight" series.

"I don't think I've ever been swept up in something like I was swept up with that," James said Thursday before her Comic-Con appearance in San Diego. "I read them and reread them and reread them, and then I sat down and wrote a novel."

That book hasn't been published ? yet ? but she followed by posting new pieces online. Those stories found an instant audience and eventually became "Fifty Shades of Grey."

Thursday's visit wasn't the London-based writer's first time at Comic-Con. She actually had come once before to participate in a panel for fans who wrote fiction inspired by the "Twilight" series.

Now, she's the bestselling author of her own literary phenomenon ? a romantic trilogy that commanded a seven-figure publishing price, has been translated into 42 languages, is dissected by book clubs and talk-show hosts and set to be made into a movie by the Oscar-nominated producers of "The Social Network."

"It's just crazy," James said, sipping a latte on the patio of a hotel near the San Diego Convention Center. "This whole thing has just been mindboggling, how it's happened and how it's exploded, and so quickly."

The "Fifty Shades" books, which follow the relationship between college student Anastasia Steele and billionaire-bachelor-with-a-taste-for-bondage Christian Grey, were published in April by a division of Random House, Inc. and have already sold more than 16 million copies.

So what's the appeal of the titillating tale?

"It's a fantasy novel," James said. "It's a love story at its heart, which I think women like to read ? a passionate love story. It's got some kinky sex in it, which is kind of interesting. ... I mean he is just ridiculously wealthy, ridiculously accomplished, as well, and it's nice to go on a holiday and just escape into their world."

As 30-year-old fan Jennifer Norling of San Francisco put it: "There's humor, there's love, it's intense. It's pretty much everything a woman would want in her love life."

But, please, don't call it "mommy porn."

Though James said she doesn't want to be "a social commentator," she isn't a big fan of the term some journalists and pundits have used to describe her books' erotic content and female fan base.

"I think it is disparaging. It's actually quite misogynistic," she said. "Women like sex. If it's done well, it's really quite good fun."

Also fun for fans of the book ? and James herself ? is speculating who will play the central characters on screen. Readers share endless ideas on Twitter, and while James finds some of their suggestions perplexing, she hasn't ruled anyone out. She also betrays no favorites.

Besides, she's still not sure how involved she'll be in the production or if she might contribute to the screenplay. Universal Pictures and Focus Features announced producers Michael De Luca and Dana Brunetti earlier this week, "so it's like, what's next?" she said.

She almost didn't want to do the movie at all.

"Books are such a personal experience for every individual reader," she said. "I just thought it's going to be really hard to do."

But she owed it to herself ? and to her story ? to go for it.

"If the book says anything, it's life's not a dress rehearsal," she said. "Just go and do stuff. I'm trying to embrace that."

Family of woman shot at cop's party looking for answers ...

DETROIT ? Police say the position of Adaisha Miller?s body ? she was on her knees as she danced at a party on Detroit?s west side Sunday ? explains how she was shot in the chest when an off-duty Detroit police officer?s gun went off.

But the young woman?s mother said that account is fishy and differs from what she initially heard from witnesses at the hospital.

Miller was dancing behind Officer Isaac Parrish at his home in Archdale and was on her knees and tugging at Parrish?s waist when his holstered gun fired, striking her in the chest, a police official familiar with the investigation said Tuesday.

The official said the angle of the gunshot is possible because Miller was not standing and described it as some type of ?exotic dance.?

Wayne County Medical Examiner Carl Schmidt said his office can?t tell the position Miller was in when she was shot but said the police version could have happened. Schmidt said the gun?s muzzle was pointed downward in such a way that the bullet went into Miller?s chest and came out of her back at a lower point.

?The angle was slightly downward,? he said of the findings of an autopsy conducted Monday.

Miller?s family members said they are hearing different accounts of what happened during the fish fry.

Yolanda McNair, Miller?s mother, said she was told by witnesses Sunday that her daughter and Parrish were side-by-side and her daughter put her arm around Parrish as she got a birthday hug from him when the gun went off.

?We can?t get her back,? she said. ?We deserve the truth.?

Miller, who would have been 25 on Monday, didn?t know Parrish and was being introduced to people at the party as she celebrated her birthday with two friends, relatives said. The Free Press has been unable to reach her friends.

McNair questioned why Miller would be on her knees dancing with a stranger and asked how someone dances on their knees.

?I?m not going to say how she got shot because we don?t know,? said Eddie Miller, Adaisha Miller?s father. ?We?re trying to get some answers. That?s all were trying to do.?

McNair said she hasn?t heard from police since they told her on Sunday that they were investigating her daughter?s death.

She said it wasn?t an accident Parrish had a loaded gun at his party, and she thinks he should serve jail time.

Parrish, a 16-year veteran of the force, has been placed on administrative restricted duty. Police say preliminary findings show the shooting appears to be an accident, and Parrish did not touch his gun, which was in a soft holster on the right side of his waist.

The official said police did not give Parrish a Breathalyzer, but alcohol does not appear to be a factor. The source added there was no odor of intoxicants, the officer?s speech was not slurred .

The early police investigation shows the holster Parrish was using may have allowed the trigger of his gun to be ?manipulated? while the Smith & Wesson M&P 40 was stored.

'Modern Family' girl belts out Adele, Gotye

Aubrey Anderson-Emmons made headlines in January when her "Modern Family" character Lily blurted out a not-so-nice word (bleeped out, of course) on the show. Well, here's a little sugar to go with that spice -- her mom Amy Anderson posted this adorable video of Aubrey, 5,?getting into another great "A"-name, Adele, with "Someone Like You."

"Aubrey?is crazy into Adele right now which I love," wrote Anderson on the video's YouTube page. "It's so cute hearing her belt her songs in the car. This is where she likes to sing the most. At first she didn't know if she wanted to learn this song because she thought it was too slow, but now it's one of her favorites."

A video of Aubrey singing shouldn't be too surprising; her ABC bio?reads in part, "She has rather unique musical tastes for a preschooler, and loves the Jackson 5. Her eyes light up at the mention of Michael Jackson's 'Thriller,' to which she can perform the dance moves and lyrics."

No video evidence of that exists on Amy's YouTube?channel, but if you like this video, you might also enjoy Aubrey getting into Gotye's "Somebody That I Used to Know" and another Adele track, "Rolling in the Deep."

In 1962 a small spherical satellite weighing about 77 kilograms was launched from Cape Canaveral. ?Its name was Telstar 1, and it was the first commercial telecommunications satellite?the first of a long line that have led to today's digitally connected world, where television programs and other media are easily accessible at locations across the globe.

By the following February, however, Telstar 1 had been completely fried by energetic electrons from a U.S. high-altitude nuclear test.

Walter Brown, a Bell Laboratories engineer who worked on the project, recalls Telstar 1?s triumphs and untimely demise. Currently a professor of materials science and engineering at Lehigh University in Pennsylvania, he says it was his job to ?examine how radiation in space affects solar cells and semiconductors.? He got rather more than he bargained for.

The day before launch, the U.S. had set off a nuclear explosion at an altitude of 400 kilometers just southwest of Johnston Island in the Pacific Ocean. The test, known as Starfish Prime, released the energy equivalent of 1.4 megatons (million tons) of TNT?creating a huge electromagnetic pulse that produced spectacular aurora over the Pacific.

"The people who set off the nuclear explosion were totally surprised by the huge number of high energy electrons that were released," Brown says. "They had no idea this would be the case until we started seeing this huge flux, a hundred times what was predicted."

The satellite unwittingly became an experiment to analyze the aftermath of a nuclear blast on electronic equipment. "We learned a lot about radiation damage from Telstar 1," he says. "Initially, Telstar 1 couldn't be turned on, some transistors had failed. But the electronics engineers figured a way around that and got it working."

Their efforts bought enough time for the satellite to prove its worth. On July 11, 1962, a day after launch, Telstar 1 relayed the television transmission of an American flag, located outside a base station in Andover, Maine, to a station in Pleumeur-Bodou, France. Brown remembers what happened at the Andover station when the satellite was turned on and radio transmission commenced: ?The project leader Eugene O?Neill whooped and gave thumbs up. And soon everyone was whooping and giving thumbs-up."

On July 23, 1962, Telstar 1 relayed a public broadcast featuring Walter Cronkite, a baseball game, and segments of a news conference by President Kennedy. That evening, it transmitted the first non-cable phone call across the Atlantic.*

Telstar 1 vindicated the vision of John Robinson Pierce, a famous Bell Labs engineer who had calculated that 25 satellites placed in suitable orbits around the Earth could provide continuous communication between any two points on the globe by bouncing signals. The first test of his idea had been the Echo 1 satellite, a giant 30-meter-diameter balloon coated with a metallized film, which NASA launched in 1960. Known as a passive communications satellite because it carried no electronics but rather acted as a giant signal reflector, it was used by Bell Labs engineers to successfully bounce telephone, radio and television signals off it. Telstar 1 went a step further. It had its own power source--solar cells that generated approximately 14 watts of power, and a transponder to receive and retransmit television signals or telephone calls.

Its success against the odds inspired a generation of scientists and engineers. Louis Lanzerotti, a physicist at New Jersey Institute of Technology who spent many years at Bell Labs and worked on space missions such as Voyager, Ulysses and Galileo, was a graduate student in nuclear physics at Harvard University when Telstar 1 went into orbit. "The graduate students in the cyclotron lab talked about it," he recalls. "We talked about sending signals across the Atlantic."

China's Inflation Eases

BEIJING?China's consumer inflation slowed sharply to the lowest level in nearly two-and-a-half years in June, likely one key reason why the central bank was comfortable with cutting benchmark interest rates for the second time in less than a month last week.

Economists said the slower inflation is opening doors for a further loosening of monetary policy and more investments by the government in the second half to ensure the country's economy rebounds.

(AP) ? An insurgent bomb killed six American soldiers in eastern Afghanistan, part of a surge of violence that has dealt a stark reminder that war is still raging as NATO plots an exit strategy for the end of 2014.

Violence raged across the country Monday, including grisly attacks by multiple suicide bombers, a day after donor nations meeting in Tokyo to pledge $16 billion in aid.

President Hamid Karzai and his top ministers said the pledges exceeded their expectations and sent a strong signal that the international community will not abandon Afghanistan even though foreign troops have started to leave.

It also sent a message to Karzai's adversaries in the Taliban and elsewhere, who are hoping his support will weaken once the foreign combat troops leave or move into support roles in 2 1/2 years.

Sunday was a particularly deadly day in Afghanistan. Roadside bombs and militant attacks killed seven American soldiers, 19 Afghan civilians and seven Afghan policemen.

Violence erupted again on Monday as militants launched suicide attacks on two police headquarters and carried out other assaults that left 20 people dead ? three policemen, an Afghan prosecutor, two children and 14 attackers, according to officials. At least 60 other people were wounded.

German Brig. Gen. Gunter Katz, a spokesman for the U.S.-led coalition, said the six Americans were killed Sunday when their armored vehicle struck a bomb in eastern Afghanistan. He said a seventh American soldier was killed in a separate insurgent attack Sunday in the south.

Taliban spokesman Zabiullah Mujahid claimed responsibility for that attack in Wardak province, just south of Kabul. Coalition and Afghan forces are trying to secure areas of Wardak that insurgents use as gateway into the Afghan capital, where they have staged high-profile attacks on Afghan government and NATO targets.

Monday's violence began when gunmen assassinated a chief prosecutor in eastern Ghazni province as he drove to work. Mohammad Ali Ahmadi, the deputy provincial governor, said Sahar Gul was shot twice.

Later in the day, three suicide bombers riding in a three-wheeled vehicle blew themselves up in Kandahar city, said Kandahar provincial spokesman Ahmad Jawed Faisal.

Then nearly a dozen other suicide attackers tried to storm the police headquarters in Kandahar, but they failed to enter the compound, Faisal said. The incident was still being investigated, but Faisal said authorities suspect that the three attackers in the vehicle, a form of miniature pickup known as a Zaranj, were headed toward police headquarters when their explosives detonated prematurely.

Three policemen and two children were killed in the attack. Another 18 police and 12 civilians were wounded.

"I'm not happy to sit here and announce such bad news, but we are facing an enemy that doesn't have a heart, so we have to be strong," Kandahar provincial Gov. Tooryalai Wesa told reporters at a news conference. "They don't care about the innocent people. They just want to scare the people."

A total of 14 suicide attackers, who fired at police for about two hours from several directions, blew themselves up or were shot and killed by police, Kandahar officials said.

Militants also attacked a police headquarters building in Shibirghan, the capital of Jawzjan province in the relatively peaceful north.

Provincial governor Mohammad Aleem Saaie said a suicide bomber on a bicycle blew himself up near the headquarters. No one died, but he said 26 people were wounded, including two policemen, a doctor and a prosecutor.

"This was another attack against innocent civilians," said Gen. Abdul Aziz Ghairat, the provincial police chief. "The majority of the wounded people are civilians."

Attackers have picked up the pace during the summer months, planting improvised explosive devices, or IEDs, along roads or footpaths. But during the past year, U.S. troops found and avoided more homemade bombs than a year ago because of improvements in training, equipment and intelligence, the U.S. military said.

In the first three months of this year, 5 percent of the bombs planted across Afghanistan hit their mark, according to the Pentagon's Joint IED Defeat Organization. That's down from 10 to 12 percent over the same three-month period a year ago.

So far this year, 231 U.S. and other NATO forces have been killed in Afghanistan. That compares with 271 in the first six months of last year.
